Summary: Objective To evaluate the value of using 18F-FDG PET/CT imaging in combination with serum lactate dehydrogenase (LDH) for predicting the prognosis of newly diagnosed small cell lung cancer (SCLC). Methods We reviewed 118 patients with pathologically proven SCLC who underwent 18F-FDG PET/CT imaging evaluation in our hospital. Among these patients, 54 patients were limited disease (LD), and 64 patients were extensive disease (ED). The maximum standardized uptake value (SUVmax) of primary tumor was measured. A Cox proportional hazards model was used to evaluate age, sex, performance status, lactate dehydrogenase (LDH), tumor stage and SUVmax on the prediction of overall survival (OS) and median survival time (MST) of patients. Subgroup analysis was performed based on the SUVmax in combination with LDH. Results According to the ROC curve, the optimal cut-off value of SUVmax was 10.95. The AUC was 0.535 (95% CI: 0.407 ~ 0.663). The patients were divided into four groups according to the SUVmax (cut-off 10.95) and LDH (cut-off 245 U/L). In LD. There was no significant difference in MST among the different groups (p =0.057). In patients with ED, there was no significant difference in MST among the different groups (p =0.742). Among all patients, 27 had low SUVmax and normal LDH, and their MST was 36 months (95% CI, 12.98-59.02); 91 patients had high SUVmax and/or high LDH, and their MST was 20 months (95% CI, 15.47~24.53). The difference between these two groups was significant (p =0.045). In patients with LD, 16 patients had low SUVmax and normal LDH, and their MST was 72 months (95% CI, 26.00 ~118.0); 38 patients had high SUVmax and/or high LDH, and their MST was 27 months (95% CI, 20.80~33.21). The difference between these two groups was significant (p=0.012). Conclusion 18F-FDG PET/CT in combination with serum LDH were prognostic factors of overall survival of SCLC patients. The prognosis of patients with LD SCLC who had low SUVmax of the primary tumor and normal LDH was better than that of patients with high SUVmax and/or high LDH.

Summary: We aimed to evaluate the correlation between 2-deoxy-2-[18F]fluoro-D-glucose (FDG) uptake and disease activity assessed by serum inflammatory biomarker levels in patients with spondyloarthritis (SpA). A total of 36 SpA patients (24 untreated and 12 treated) were examined using FDG positron emission tomography (PET)/computed tomography and classified into axial SpA (axSpA) and peripheral SpA (pSpA). FDG uptake was evaluated in 23 regions of the body and scored as follows: 0 = less than liver uptake; 1 = more than or equal to liver uptake; and 2 = more than or equal to twice liver uptake. A score of 1 or 2 was considered positive. The number of positive regions and the total score were counted in each patient. The maximum standardized uptake value (SUVmax) was calculated for each region, and maximum SUVmax (MaxSUVmax) was used as a representative value. Correlation of PET findings with serum inflammatory biomarker levels, including C-reactive protein (CRP), erythrocyte sedimentation rate, and matrix metalloproteinase 3 (MMP-3), was analyzed. All but two patients had at least one positive lesion. PET indices correlated significantly with most of the serum inflammatory biomarker levels in untreated SpA, but not in treated SpA. Further, MaxSUVmax, number of positive regions, and total score correlated significantly with CRP (all P values < 0.001), and the number of positive regions (P = 0.012) and total score (P = 0.007) correlated significantly with MMP-3 in untreated pSpA. PET indices did not correlate with any serum inflammatory biomarker level in untreated axSpA. FDG uptake in untreated pSpA correlated significantly with serum inflammatory biomarker levels.

Summary: Purpose To perform a systematic review of the effect of blood glucose levels on 2-Deoxy-2-[18F]fluoro-D-glucose (18F-FDG) uptake in normal organs. Methods We searched the MEDLINE, EMBASE and Cochrane databases through 22 April 2017 to identify all relevant studies using the keywords “PET/CT” (positron emission tomography/computed tomography), “standardized uptake value” (SUV), “glycemia,” and “normal.” Analysis followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses recommendations. Maximum and mean SUVs and glycemia were the main parameters analyzed. To objectively measure the magnitude of the association between glycemia and 18F-FDG uptake in different organs, we calculated the effect size (ES) and the coefficient of determination (R2) whenever possible. Results The literature search yielded 225 results, and 14 articles met the inclusion criteria; studies included a total of 2714 (range, 51–557) participants. The brain SUV was related significantly and inversely to glycemia (ES = 1.26; R2 0.16–0.58). Although the liver and mediastinal blood pool were significantly affected by glycemia, the magnitudes of these associations were small (ES = 0.24–0.59, R2 = 0.01–0.08) and negligible (R2 = 0.02), respectively. Lung, bone marrow, tumor, spleen, fat, bowel, and stomach 18F-FDG uptakes were not influenced by glycemia. Individual factors other than glycemia can also affect 18F-FDG uptake in different organs, and body mass index appears to be the most important of these factors. Conclusion The impact of glycemia on SUVs in most organs is either negligible or too small to be clinically significant. The brain SUV was the only value largely affected by glycemia.
