Self- or interviewer-administered questionnaires can be used to measure cross-sectional differences in quality of life among patients at a point in time (discriminative instruments) or longitudinal changes in HRQL within patients over time (evaluative instruments). Both discriminative and evaluative instruments must be valid (ie, measure what they are supposed to measure) and have a high ratio of signal to noise (reliability and responsiveness for the two instruments, respectively). Reliable discriminative instruments are able to differentiate reproducibly among persons. Responsive evaluative measures are able to detect important changes in HRQL over time, even if those changes are small. HRQL should also be interpretable B that is, clinicians and policy makers must be able to identify differences in scores that correspond to trivial, small, moderate and large differences.There are two basic approaches to quality of life measurement: generic instruments that attempt to provide a summary of HRQL and specific instruments that focus on problems associated with individual disease states, patient groups or areas of function. Each approach has its strengths and weaknesses and may be suitable under different circumstances. Current regulatory considerations are discussed and resources outlined. Quality of life may be the most important endpoint for patients with rare diseases, and the challenges of valid assessment require effort and innovative thinking specific to each context to improve measurement and clinical outcomes.

Summary: Author summary Dengue, a viral illness occurring following the bite of an infected mosquito, is common in tropical urban areas of the globe. Using a survey that enquires about not only physical health but also psychological health, self-care, mobility and usual social activities, this study assesses the overall impact of dengue on the well-being of those with the disease. Of the estimated 390 million annual infections, approximately 75 percent do not cause individuals to attend medical facilities, possibly because many infections do not result in significant symptoms or because access to healthcare is limited. Unlike previous studies, this study recruited those who would have been unlikely to seek healthcare as well as individuals who attended health facilities. This study adds to the growing body of evidence that healthcare professionals should regard dengue as a disease that affects not only physical, but also psychological health and aspects of social functioning. Furthermore, it suggests that those not seeking treatment may also have a substantial reduction in quality of life during their infection. Given the millions of dengue infected people who do not attend healthcare facilities annually this could amount to a very meaningful societal impact.

Summary: The ability to objectively measure aspects of performance and behavior is a fundamental pillar of digital health, enabling digital wellness products, decentralized trial concepts, evidence generation, digital therapeutics, and more.
